Firefighting CH-47D crashes into the Salmon River over an iPad

On 21 July 2022 a Rotak Helicopters CH-47D fighting the Moose Fire crashed into the Salmon River near North Fork, Idaho, killing both pilots. The NTSB blamed an unsecured iPad that jammed the copilot's left pedal.
