Falcon 1: failure on maiden flight due to a fuel leak

On 24 March 2006 the Falcon 1 made its maiden flight from Omelek Island, Kwajalein Atoll, carrying the US Air Force's FalconSAT-2 satellite. A fuel leak in the first-stage plumbing caused a fire and the loss of the vehicle seconds after liftoff. A later DARPA review attributed the leak to a fuel-line nut that failed due to saltwater corrosion.
